East Glasgow Community Health & Care Partnership Service Information East Glasgow Community Health and Care Partnership (CHCP) manages health and social care services in east Glasgow. The CHCP is a partnership between Glasgow City Council and NHS Greater Glasgow and Clyde to deliver social and health care in community settings such as local health centres, clinics, social work offices across East Glasgow. The CHCP is not just about the delivery of local health and social care services, our aim is to improve the health and well-being of the residents of east Glasgow. To do this we work with local community planning partners, such as the Glasgow City Council Community Planning, Glasgow East Regeneration Agency, Glasgow Community Safety Services, Culture and Sport Glasgow, John Wheatley College and many local voluntary and community groups. We also make sure that the people who use our services, carers, members of the public, community and voluntary bodies and staff have a voice in the design and delivery of services through our Public Partnership Forum (PPF). These include assessment and care management, child protection, residential and foster care, support for young people leaving care, specialist services for children affected by disability, support to carers, support for young people with training and employment, health based social care services, youth justice, community and individual support services. PACT are a Team of CHCP staff from the health service, voluntary sector and social work, supported by education services. The PACT Team works with a variety of parents at different times in their lives. For example it may work with a mother from before the birth of her baby or with parents at any time until the child reaches the age of 5 years. The team offers support in many ways. It will work with parents to agree the sorts of supports that are best for at the time. Some of the activities Pact can help with are –developing parenting skills, advising on money issues, helping access to training and employment, helping clients to cope with stress and develop healthier lifestyles. Respite carers : Look after children in their own homes for short periods, normally just a few days. Respite carers provide children and families with valuable breaks which can often relieve difficulties and prevent family situations from breaking down. To find out more, or if you are interested in becoming a respite carer, contact Respite Care Team Phone 0141 302 2784. Supported carers : Offers accommodation with support to young people who are, or have been, looked after by Social Work Services. To find out more, or if you are interested in becoming a supported carer, contact Leaving Care Services Phone 0800 027 7954. Criminal Justice services: Services are provided on behalf of the Scottish Executive. These include assessment and reports, probation, community service orders, drug treatment and testing orders, through care, services to the court system and prison based services. Podiatry (foot care) is available in all health centres and clinics. A home visiting service is available for patients who are totally housebound. Free internet access is also provided. All services are free of charge and available without the need to be referred by your doctor or another practitioner.
